Requesting Your Health Care Records from Mesa View Counseling and Consulting

To request a copy of your health care records, please submit a written request to:

  • Date of birth

  • Phone number and/or email address

  • Dates of service being requested

  • Where you would like the records sent

Please attach a copy of a valid photo ID to your email request.
